- Certificate of Insurance Naming "Freight Management Logistics, LLC 4585 CR 141 Wildwood, FL 34785" as the certificate holder - with original certificate to follow. WE MUST RECEIVE ORIGINAL CERTIFICATE VIA MAIL FOR PAYMENT AUTHORIZATION.
$1,000,000 in Auto and General Liability minimum.
$100,000 Cargo Minimum.
